You are permitted to file Form I-102, “Application for Replacement/Initial Nonimmigrant Arrival-Departure Document.” This Form can be found on the USCIS website. The current filing fee for Form I-102 is $445.00, and there is no biometrics fee associated with this Form.
The filing fee may be waived in certain limited situations. A Replacement I-94 Arrival/Departure Form may be necessary if you are required to establish the time, place, and manner of your prior admission to the U.S.
